Daycare/School/Camp Forms

In an effort to reduce VAPG’s carbon footprint, we are going green with forms. All forms completed at the well check will be available on the patient portal by the end of the day.

Free: Forms will be completed for free if brought with the child to the Well-Check visit and uploaded to the patient portal on the same day of the appointment.
If you would like the form printed by one of our offices, there will be a charge of $10.00.

If you request a form on a day other than the Well Check appointment.

Forms will be available in 7-10 business days, there will be a $15.00 fee, and the form will be provided on the patient portal unless specifically requested on paper.
If you would like the form printed by one of our offices, there will be a charge of $10.00.
If you request a same-day form completion outside the Well Check appointment, there will be an additional rush fee of $25.00.
